The Round Robin format will be limited to 10 minute games or first to 11. The top two point gathering teams will qualify for a championship playoff immediately following.
Making A Difference
At our U CAN-CER VIVE Dreamy Evening for the Cure on Friday, June 2, 2023 we honored and recognized individuals who are making a difference and an impact in our community and in the world of cancer.
We can’t thank our sponsors, donors, volunteers and supporters enough for their unwavering support. We are proud to announce that we raised over $500,000 towards vital funds for cancer research. What’s even more amazing is that 100% of profits raised to our cause! We couldn’t do it without your support, so thank you from the bottom of our hearts.
During this magical evening, we took a special moment to honor two individuals who are making strides in the cancer community. One of which includes a hospital partner who has poured her heart and passion into her work and into our community. Linda Filipczak from Barbara Ann Karmanos Cancer Institute has been making strides in the cancer community, along with helping U CAN-CER VIVE to fulfill our mission. We have been proud to support the research at Karmanos, and Linda has been a helping hand along the way! During our Evening for the Cure program, we honored Linda with our Community Partner Award for 2023. She is so deserving!
As a committee, we aim to lead with our hearts, inspire one another, and spread the word about cancer research within our community. As a partner with C.S. Mott Children’s Hospital and Michigan Medicine, Davis Warren, a football player and student at Michigan University, was kindly introduced to us in early 2023. We realized how powerful and meaningful the connection was when Davis shared his story of battling Leukemia during his high school football years…
If you can’t find him on the field, you can find him volunteering his time bringing awareness to the same thing we are all supporting; cancer research funding. We are so proud to share that Davis Warren will be our 2023-2024 U CAN-CER VIVE Ambassador! We look forward to sharing his journey with you all and feel honored to have Davis represent for himself, the cancer community, and for our foundation.
Dream Raffle Rules and Release
Tickets for the Dream Raffle (the “Raffle”) may be purchased for $250 beginning at 9 AM EST on Tuesday, December 13, 2022 through 10:00 PM EST June 2, 2023 (“Raffle Period”). By purchasing a Raffle ticket, each entrant unconditionally accepts and agrees to comply with and abide by the rules, terms, and conditions below (the “Raffle Rules”), and the decisions of Team LaFontaine Ucancervive Foundation (“UCCV”), which will be binding in all respects.
ELIGIBILITY
The Raffle is open to any individual who is eighteen years of age or older, and a Michigan Resident, and a U.S. Citizen as of the date that the Raffle ticket is purchased, and who abides by the Raffle Rules. Prior to awarding the Prize to the winner, each entrant must show proper identification and provide their driver’s license and social security number verifying their identity. Purchase of a Raffle ticket by more than one individual, purchase on behalf of an entity, and purchase on behalf of another person(s) are not permitted.
PRIZE
UCCV will sell raffle tickets on a first-come, first-served basis. The winner of the Raffle will receive title to the 1975 Ford Bronco Sport 4×4 SUV (VIN U15GLW65900) (the “Prize”). The Raffle winner may, in the alternative, elect to obtain the cash prize of $10,000 (the “Alternative Prize”). The Raffle winner must notify UCCV of its decision on whether to accept the Prize or the Alternative Prize within 48 hours of the end of the Raffle Period.
The winner may not elect to assign the Prize except at the discretion of UCCV. Liability for any applicable federal, state, or other taxes, or any other costs, will be the sole responsibility of the winner and no prize will be awarded without receipt of the proper legal paperwork as determined by UCCV. The approximate fair market value of the Prize is $75,000. The actual value may vary depending on the time that the Prize is awarded and UCCV shall solely determine the fair market value at the time the Prize is awarded, which value shall be used to determine the applicable taxes that must be paid by the winner prior to the award of the Prize.
UCCV will not provide any warranties for the Prize. The Prize shall be awarded to winner on an “AS IS” basis and UCCV DEALER DISCLAIMS ALL WARRANTIES, EITHER EXPRESS OR IMPLIED, INCLUDING ANY IMPLIED WARRANTY OF MERCHANTABILITY OR FITNESS FOR A PARTICULAR PURPOSE.
UCCV reserves the right to substitute a prize of the same approximate retail value. In the event that UCCV elects to replace any prize or portion thereof with a substitute prize or portion thereof prior to the end of the Raffle Period, a notice shall be posted at www.ucancervive.com, indicating such change prior to the end of the Raffle Period. In the event that UCCV elects to replace any prize or portion thereof with a substitute prize or portion thereof after the end of the Raffle Period, the winner of such prize will be notified in writing of the change. No entrant shall have any claim or demand whatsoever against UCCV and the respective parents, affiliates, subsidiaries, directors, officers, agents and employees arising from or related to any prize substitution made by UCCV in accordance with this paragraph.
Raffle winner will be responsible for maintaining all insurance coverage required for the Prize, if any, and under applicable laws, and shall be responsible for any other taxes, including sales or income taxes, costs, expenses, license, title and registration fees. Winner must provide all reasonable information requested by UCCV including, but not limited to name, address, driver’s license or state identification number, telephone number and social security number. Winner must be able to register and title the vehicle as required by Michigan law.
Raffle winner must take delivery of the Prize at a location to be designated by UCCV. Any travel or transportation costs associated with winning of the Prize will be the responsibility of Raffle winner. The depiction of the Prize shown or described in any promotional materials may not reflect the actual Prize delivered to Raffle winner. All details of the Raffle Prize not specified herein shall be determined solely by UCCV. All expenses relating to acceptance of the Prize, which are not explicitly listed herein are the responsibility of the Raffle winner.
WINNER DRAWING/ODDS
The winning Raffle ticket will be randomly selected at 1 Concourse Drive, Pontiac, Michigan, Unit 133 at 8:30 pm on June 2, 2023. Chances of winning are based on the number of Raffle tickets purchased. For example, if 3,000 Raffle tickets are purchased, then chance of one ticket winning is 1 in 3,000.
The winner need not be present to win. The winner will be notified by email and phone. UCCV may publicly announce the winner through social media channels, including, but not limited to, UCCV’s Website, Facebook, Instagram, LinkedIn page, etc. Winner may receive notification by email and phone, prior to, concurrent with, or subsequent to, any such social media announcement. Any prize not claimed within 60 days from the drawing date must be awarded using another drawing of the original ticket stubs or donated to another non-profit with permission from the Charitable Gaming Division. Prizes cannot be reverted to the licensed organization.
As indicated below, the winner must sign a release and consent form that allows UCCV to use the winner’s name, image, and likeness in UCCV advertising. If UCCV does not receive the completed release and consent form within 10 calendar days of the date the winner was first called or emailed about their prize, a new winner may be selected.
HOW TO ENTER
Raffle tickets may only be purchased in-person at a location where UCCV is conducting Raffle ticket sales.
Raffle tickets are nonrefundable. UCCV is not responsible for:
- Lost Raffle tickets;
- Attempts to purchase a Raffle ticket outside of the Raffle Period; or
- Mistaken, incorrect, or illegible contact information provided to UCCV, resulting in UCCV’s inability to contact the owner of a winning Raffle ticket;
GENERAL RELEASE
By entering this Raffle, each entrant (i) accepts and agrees to these Rules and the decisions of UCCV which shall be final and binding; and (ii) waives any right to claim ambiguity in the Raffle or these Rules. Each winner will be required to execute and return an affidavit of eligibility, a liability release, a publicity release, and any other forms requested by UCCV within 10 days of notification. In the event of non-compliance or if a winner of a prize cannot be located, that winner shall forfeit the prize and an alternate winner of the prize shall be selected from the original pool of raffle tickets. By entering, each entrant releases and holds harmless any person or entity associated with the production, judging, sponsorship, advertising or administration of the Raffle, including, without limitation, UCCV and the respective parents, affiliates, subsidiaries, directors, officers, agents, and employees of any of the above organizations (collectively, the “released parties”) from any and all liability arising from, or related to participation in the Raffle or the acceptance of the Prize or Alternative Prize. Except where prohibited by law, the winner’s purchase of a Raffle ticket and acceptance of a prize constitutes permission for UCCV to use the winner’s name, photograph, likeness, statements, biographical information, voice, and city and state address for promotional or advertising purposes in connection with this Raffle and similar promotions on a worldwide basis and in all forms of media, now or hereafter known, in perpetuity, without review, permission or other compensation.
GENERAL CONDITIONS
UCCV reserves the right to modify, suspend, and/or terminate this contest for any reason in its discretion, including if UCCV determines in its sole discretion that a virus, bug, nonauthorized human intervention, or other cause corrupts or impairs the administration, security, fairness, or resolution of the Raffle. In case of suspension or termination, a notice will be posted, and, at UCCV’s discretion, Raffle tickets may be refunded or a winner may be selected from all non-suspect Raffle tickets purchased. Inclusion in such alternate raffle drawing is not guaranteed, but if it occurs will constitute an included entrant’s sole and exclusive remedy under such circumstances. UCCV reserves the right to disqualify any entrant if his or her fraud or misconduct affects the integrity of the Raffle or fails to remain in good standing with UCCV.
UCCV retains the right to change or modify these rules at any time without notice. UCCV is not responsible for any typographical or other error in the printing of Raffle materials, administration of the Raffle, or in the announcement of the prize. By participating, entrants agree to these Raffle Rules and to the determination of the Raffle winner, which will be final and binding in all respects.
Any claim or controversy between any participant in the Raffle and UCCV arising out of or relating to the Raffle or these rules and regulations shall be settled by binding arbitration administered by the American Arbitration Associationutilizing one (1) arbitrator to be heard in Southfield, Michigan unless other procedures are agreed upon in writing between the Parties. The determination of the arbitrator in connection with interpreting these Rules shall be binding and final upon all Parties. The award of the arbitrator may be filed with the clerk of the appropriate court having jurisdiction in Oakland County, Michigan. Judgment may be rendered by the court upon the arbitration award and execution may be issued upon the judgment in accordance with the statutes of the State of Michigan and the Michigan Court Rules. Any legal challenge related to the Raffle must be filed within 6 months of the determination of the Raffle winner. Such legal challenge will be waived if not filed within such period. Participation in the promotion constitutes a participant’s agreement to this 6-month limitation period.
The invalidity or unenforceability of any provision of these Raffle Rules shall not affect the validity or enforceability of any other provision. In the event that any provision is determined to be invalid or otherwise unenforceable or illegal, these Raffle Rules shall otherwise remain in effect and shall be construed in accordance with their terms as if the invalid or illegal provision were not contained herein. The Raffle and all issues relating to it and these Raffle Rules will be governed by and construed in accordance with the internal laws of the state of Michigan without giving effect to any choice of law or conflict of law rules or provisions that would cause the application of any other state’s laws.
The license number for this raffle issued by the Charitable Gaming Division of the Michigan Bureau of State Lottery (“Bureau”) of the State of Michigan to Team LaFontaine Ucancervive Foundation (“UCCV”) is R67627. All net proceeds of this Raffle shall be retained by UCCV and used in a manner supporting its charitable purpose as provided in its governing documents as a Section 501(c)(3) charitable organization. No refunds shall be allowed, except in the event the raffle ticket is not accepted by UCCV or if the Raffle is not or cannot be conducted as scheduled and an alternate raffle is not planned, at the sole discretion of UCCV. In the event of any conflict between these Rules and the rules, regulations, and directives of the Bureau, the rules, regulations, and directives of the Bureau shall control. This Raffle shall in all respects be conducted in accordance with the Michigan statutes and the rules, regulations, and directives of the Bureau.
